To do that, open up the savegame with Notepad++, then use the replace command to change all of the unconverted provinces to the reformed faith. After you have all 5, you can instantly reform, though you may want to edit the savegame to clear up the unreformed provinces, which will port to EUIV as generic animist. You can instantly locate the temples by clicking on the 5 slots for them in the religion tab. Usually, the code-name of baronies is identical to their in-game name, so just put that in after the b_ and put your character's id in the second slot, and repeat for all 5 holy sites. It should be "give_title charid b_temple". Just give a pagan character all of the required temples with the console.
